An Evaluation of the Caring Communities Initiative of the Leaders Roundtable
The Caring Community evaluation focused on understanding the development and outcomes of eight community-based teams designed to improve systems integration and community cohesion. The evaluation focused on understanding organizational effectiveness of the community groups, as well as outcomes related to systems and service integration, community involvement, and other key outcomes.
